Why does the Trans not shift out of first?

The engine runs great, but the trans is not shifting right. When I am driving normally, it won’t shift out of first until about 2300rpm and then clunks into 2nd. If I am hard on the accelerator, it does the same from 2nd to 3rd, basically missing the shift point.
