Care Coordination for Children (CC4C) is a new program for eligible children from birth to age 5. CC4C works to improve the overall health of young children. If your child qualifies for this program, your family can receive services offered by CC4C. CC4C focuses on family strengths and concerns.

Formerly Child Service Coordination (CSC) As of March 1, 2011, the Child Service Coordination Program has changed to a new service called Care Coordination for Children (CC4C).
What is care coordination kids?
California does not spend what it would take to meet the needs of children. Care coordination refers to efforts by Medicaid programs to ensure that children and adolescents get the right care at the right time in the right setting by creating a bridge across multiple systems that serve children and families.
