Why does my Ford Focus rev up and down?

Idle Air Control Valve maybe.. Since it only happens when the car is hot, EGR valve gummed up and sticking open? I’m going on the logic that revs bouncing up and down could be a vacuum leak, but i can’t think of anything else that comes in to play only when the cars warm.
