If you need to pump your clutch to make it work properly then you have a problem (almost 100% certain) with either air in the line OR you have a slave cylinder on the way out. It is not a transmission fluid related problem, as they are two separate systems and completely sealed away from each other.

Yup, happened to my MK6. Ford Australia told me that my 2004 Fiesta Zetec missed the recall, so i had to pay up and get a new master cylinder for the clutch as the lever broke. Two months later it started falling off again, they put a pin in and said goodbye. ! Removed!. 3 weeks later…

You say there are no visible fluid leaks but the slave cylinder (the part that operates the clutch) is inside the bell housing on the gearbox so a fluid leak there would not be visible.

What causes a petal to fall out of the clutch?
Check linkage. sometime the rod that pushes the t/o rod fork will fall out of the rod. It’s not attached, held in place only by spring pressure. Easy fix if that’s the case, caused by out of adjustment. Good luck , hope that’s all it is. Worst case all pressure plate componets destroyed and nothing to give petal something to push
