Why is my rear vent window not closing?

The vast majority of the time it makes no noise at all when I move the switch, but a couple of times I’ve heard a very weak grunt from the motor. From the schematics it looks like these motors have PTC fuses builtin (to prevent the motor from overheating due to overuse or misuse), in addition to the main PTC’s down by the BCM.

Where is the rear vent window on a minivan?

Grab the piece of trim at the point where it overlaps the trim in the rear corner of the minivan. There will be a flange that overlaps the speaker grille trim with the body trim. Get your fingers under that flange and give it a good yank. That should do it.
